Attention all staff,

​With the flu season nearly approaching we are needing the following issues to be resolved.


1: Making sure your Provider is the GP and not the nurse. This error cannot be manually fixed and is time consuming having to fax errors back to the clinic to be corrected.

2: Letting NIR know if you have a new GP or nurse joining your practice even if they are locums.

Your assistance to make us all more efficient is much appreciated.

National guidance on type 2 diabetes

​National Guidelines on the management of Type 2 diabetes.  NZSSD guidelines on all aspects of the management of type 2 diabetes is now available at www.t2dm.nzssd.org.nz . It is designed to be concise pragmatic evidence-based advice with no reinvention of the wheel. Some of the highlights of the guidance includes (in no particular order):
 
  •          Shifting from a ‘glycaemic control  based’ treatment algorithm to one reducing cardiovascular and renal disease
  •          That the target HbA1c for most patients is < 53 mmol/mol
  •          When to screen for diabetes in asymptomatic adults
  •          When and how to use SGLT2 inhibitors and GLP1 receptor agonists
  •          When and how to use other glucose lowering therapies including ‘capping’ basal insulin at 0.5 mg/kg/day before introducing prandial insulin
  •          Reducing clinical inertia and how to act on abnormal findings in the ‘annual diabetes review’

The NZTLG National Telehealth Research, Audit, Evaluation and Resource Registration Project

As part of understanding what has been going on across the country to develop telehealth services, The New Zealand Telehealth Leadership Group (NZTLG) has launched a National Telehealth Register to collate and disseminate information on current or completed work undertaken on telehealth.
 
Please find attached 1. a letter from myself, NZTLG Chair, Dr Ruth Large and NZ Telehealth Forum: Research, Audit and Evaluation Working Group Chair, Associate Professor Inga Hunter that outlines more about the Register; and 2. the Registration Form.

We would like to ask for your assistance by circulating through your networks and registering any telehealth work by filling out the attached form and sending back to us at research@telehealth.org.nz. We will categorise to a searchable database and publish on the Telehealth Resource Centre: www.telehealth.org.nz for the benefit of all.

Vacancy - Hauraki PHO - Outreach Immunisation Nurse 0.8FTE

REGISTERED NURSE- OUTREACH IMMUNISATION MOBILE CLINICAL SERVICES TEAM
Hauraki PHO supports five GP practices servicing 16 clinics across  the Hauraki and Waikato rohe and  is seeking to appoint a suitably qualified person to fulfil the above role for our enrolled population in the Waikato rohe.
The successful applicant will,   provide support to the Outreach Immunisations Team, providing a hands on, mobile service, and focusing on improving the health and wellbeing of babies and children.

The successful applicant will be an experienced registered nurse with a minimum of three years community and /or primary health care experience.  We are looking for a reliable team player who can demonstrate high levels of initiative, flexibility, leadership, with highly developed interpersonal skills.  Candidates need to have an ability to work well on all levels to support whanau, in collaboration with Hauraki PHO staff and HPHO GP practice team partners.
To succeed in this appointment you must have a commitment to improving the health outcomes of all those experiencing health issues who are enrolled with Hauraki PHO practices.

​Key competencies of this position include
:
  • Registered Nurse with current APC
  • Independent vaccinator Certification
  • Excellent verbal and written communication skills
  • Confidence to work independently in multiple community settings
  • Excellent relationship building and liaison skills
  • Computer skills, particularly Medtech21
  • Current clean driver’s licence essential.

The successful applicant will be reliable team players who have demonstrated experience delivering brief intervention,  flexibility, excellent interpersonal skills and an ability to work well on all levels to support people who experience primary mental health conditions in collaboration with Hauraki PHO staff and GP practice team partners.
 
To succeed in this appointment you must have a commitment to improving the health outcomes of all those experiencing a primary mental health issue who are enrolled with Hauraki PHO practices.
 
Key competencies of this position include:
A health professional qualification within the meaning of the Health Practitioners Competence Assurance Act 2003 or Social Work registration within the meaning of the Social Workers Registration Act 2003, recognised as health practitioners in the Health & Disability Commissioner Act 2003 Amendment No. 49
 
  • Excellent computer skills, Medtech32 an advantage
  • Excellent communication, relationship building and liaison skills
  • Experience delivering brief intervention within specified timeframes
  • Excellent written and verbal communication skills.
  • Current clean driver’s licence essential.
