Diabetes Action Month – Kei whakawā. Me Aroha. Love don’t judge.

This year Diabetes Action month places emphasis on the emotional burden that Diabetes has on whānau.

There is so much exciting action happening in diabetes around the wider Hauraki PHO rohe at the moment.
 
  • Te Rengarenga hosting a collaborative diabetes evening with the DHB team. Whānau will come for their annual review, meeting with the podiatrist, retinal screening, dietitian, nurse and pathlab. A one stop shop for their diabetes check with prizes and giveaways.  
  • Te Korowai continue to fund their own dietitian to work in the Piki Te Ora team
  • Te Kōhao has two new diabetes nurses to work in the community and they continue to run the Kimi Ora programme.
  • Tainui Waka and WDHB are working on a pilot before Christmas screening for pre diabetes followed by a wānanga for education. The hope is to roll this out in 2021 across the area.
  • We have Ryan Paul, Endocrinologist to work with us one day a week for a year. He will be available for in-service, case studies and management advice.
  • While the new diabetes medicine is on hold, this may work in favour of whānau as it is likely that the SA criteria will widen.
  • National guidelines for diabetes management are to be released prior to Christmas based on international best practice so that whānau receive similar care no matter where they are in the country.
  • Hauraki PHO continues to fund education for nurses completing the WINTEC 700 diabetes short course. If you are considering applying for 2021 please be in touch soon. We already have 3 nurses who are interested.
  • In collaboration with NHC and Pinnacle we deliver diabetes education to nurses (based on the knowledge and skills framework), Kaiāwhina and GP’s. (Dietitians and Pharmacists are most welcome!)
  • Freestyle Libre continues to be a huge benefit to patients with a number of our GP practices now funding their own to use for 2 weeks with whānau who are ready to learn about their diabetes and make lifestyle changes.
  • Insulin initiation is funded by the PHO for one initial education session and one follow up session.
 
For our team of 4500 whānau living with diabetes, we need everyone one of our Hauraki PHO team to provide support, education, and understanding.
