Quality Targets: Gap to target for Quarter 2 2018/19

• Our Smoking Brief Advice rate has continued to drop this quarter. Remember to utilise the TxT2REMIND smoking campaign HPHO have funded through version 4 and utilise Appointment Scanner for opportunistic advice. Robyn Finucane, HPHO’s Smoking Champion can provide further assistance if required. (Ph: 027 530 8003)
• As discussed previously, the Manage My Health patient portal target has increased to 40% of patients aged 16+ activated on the portal. This will increase by 10% each quarter for the rest of financial year.
