The Acute Demand Programme is designed to support General Practice in preventing admission to hospital/emergency departments by using Primary Options for Acute Care (POAC) to manage patients in their own practice or community when it is clinically safe to do so.
From July 1, 2019 Hauraki Primary Health Organisation will provide a newly held contract for POAC.
Hauraki PHOs goal is to ensure that all claims will be honoured either from the POAC budget or passed back to the practice teams to re-submit the claim under Extended Consult-Acute under the practices SIA account. HPHO will report monthly on utilisation and budget.
The clinical criteria for POAC cases have been revised and attached.
The process will be the same as you currently use.
