Hauraki PHO supports 6 GP practices and 16 clinics across the Hauraki and Waikato rohe and is seeking to appoint suitably qualified Whanau Ora kaiawhina to work alongside the Registered Nurses in the Long Term Conditions  (LTC) Team.

The successful applicants will support the LTC team to provide a mobile service, addressing the environmental, psychosocial and cultural needs of whanau and using a whanau ora approach to delivery of the Long Term Conditions Programme.

The successful applicant  will be a reliable team player who shows high levels of initiative, flexibility, interpersonal skills and an ability to work well on all levels to support  whanau in collaboration with Hauraki PHO staff and GP practice team partners.

To succeed in this appointment you must have a commitment to improving the health outcomes of all those experiencing health issues who are enrolled with Hauraki PHO practices.

Key competencies of this position include:

• Experience working with diverse communities in the Hauraki rohe
• Preferred experience in a primary health care environment
• Excellent verbal and written communication skills
• Excellent relationship building and liaison skills
• Computer skills, particularly Medtech21 OR Evolution an advantage
• Current clean driver’s licence essential.
