Breast and Cervical Screening
Breast and Cervical Screening Support to Service:
This programme supports Hauraki PHO general practice partners to locate and assist priority women to attend breast and/or cervical screening. Priority women may be overdue for screening (under-screened) or have never participated in screening (unscreened).
Our intention is to work collaboratively to engage eligible women into breast and/or cervical screening programmes.
We are happy to take referrals as soon as possible and we are keen to hear from you if there are any other ways we can assist you to support the screening requirements of your enrolled population.

Māori, Pacific and Asian populations have been identified as having lower levels of cervical screening coverage.
This programme aims to increase coverage among these patients, reducing the barrier of cost to these women, by providing practices with adequate funding to ensure the smear is provided at no cost.
Women of all ethnicities who are identified as being significantly overdue for a smear are also funded under this programme.

Hauraki PHO has approximately 4,800 enrolled patients with diabetes. Nearly twenty percent of whānau have an HbA1c that is over 80. With poorly controlled diabetes leading to a higher risk of long-term complications, there is plenty of support that whānau need to stay well.
Podiatry Services for Patients with Diabetes:
Practices may contract with a local podiatrist and claim for these services using the HPHO Podiatry Advanced Form in Medtech or Evolution.

Waikato Regional Diabetes service
The Waikato Regional Diabetes Service is only seeing patients with type 1 diabetes, patients who are insulin deficient, on chemotherapy, have established CKD3, have active foot problems, those under the age of 25, or those who are pregnant. Whānau with type 2 diabetes are managed in primary health care.

Patients must be:
- Eligible to receive publicly funded services and
- Diagnosed with type 1 or 2 diabetes and high risk foot disease as defined in the Diabetic Foot Screening and Risk Stratification Tool * and
- Be enrolled with a Hauraki PHO general practice and
- Be referred by any member of the general practice multidisciplinary team

Heart Checks
Heart disease is the most common cause of death and disability in the developed world. In New Zealand, coronary heart disease is the most common form of cardiovascular disease (coronary heart disease, stroke and blood vessel disease). Each day approximately 17 New Zealanders (one every 90 minutes) die as the result of coronary heart disease. It is the leading single cause of death in New Zealand, accounting for almost one quarter of all deaths.
Overall, Māori and Pasifika patients are more at risk of suffering from coronary heart disease.
This funded service will provide free access to Electrocardiograms (ECG) and Exercise Tolerance Tests (ETT) to high risk population groups, who cannot afford to pay for this service in primary care; with a focus on prevention and early detection of heart problems.

Ngā Kaitiaki Manawanui Whai Ora
“Pathway to Wellness Support Team”
The purpose of the Ngā Kaitiaki Manawanui Whai Ora (NKMWO) programme is to walk alongside and empower individuals, and their whānau, who have long term health conditions (LTC) to successfully self-manage their conditions.
We provide a mobile nurse and kaiāwhina partnership working together with Hauraki PHO general practice teams, community agencies and hospital based health service provider colleagues.
Patients are referred by utilising the Medtech/Evolution Outbox document called MWOK Referral.

Having a nurse and kaiāwhina partnership gives the individual/whānau support with clinical aspects and social issues that may be effecting their health.

- Development of a personalised care plan;
- Advocacy/navigation support to clinical (e.g. hospital, general practice, community) and social (e.g. WINZ, budget, housing, transport) appointments;

- Encouraging whānau involvement and support; and
- Lifestyle advice

The NKMWO model of care is patient/whānau centred and provides care that is respectful, responsive and meaningful to the individual patient preferences, needs and values. We encourage high quality and sustainable long term condition management within the community that empowers individuals and their whānau to take control of their wellbeing.

This service is free for patients enrolled with a Hauraki PHO general practice and has an expected enrolment period for each individual of up to six months to assist them to move to self-management. Outreach Immunisation Service
The Hauraki PHO Outreach Immunisation Service (OIS) covers the Waikato DHB rohe addressing inequities in health by trying to reach all tamariki who for many reasons are unable to be immunised at their GP practice in a timely fashion. Some of these tamariki may not be enrolled with a general practice. The OIS team provides advice and encourages parents to enrol their tamariki with a general practice to ensure continuity of care.
Immunisation can protect people against harmful infections, which can cause serious complications including death. Immunisation uses the body’s natural defense mechanism (the immune response) to build resistance in individuals and the community. Outreach Immunisation service information
This service reduces the impact of vaccine preventable disease by increasing the coverage (National Target 95%) and reducing inequalities in the immunisation coverage. Opportunistic immunisations are completed for tamariki who are identified as having missed vaccines from the New Zealand Immunisation Schedule.
OIS provides coordination support for the another OIS provider ( Raukura Hauora o Tainui). We work together to ensure timely immunisations for our tamariki.
The National Immunisation Schedule is the series of vaccines that are offered free to babies, children, adolescents and adults.

Primary Mental Health Services
The Hauraki PHO Primary Mental Health service is a brief intervention service offering four sessions of counselling for mild to moderate mental health conditions.
There are also funded Packages of Care available with contracted psychologists, psychotherapists and counsellors.
It is available through general practice referral and is a confidential and free service for those aged 12 years and over.

After Hours Services
HPHO practices work collaboratively with the Waikato District Health Board and local practice teams to ensure that both the primary and secondary rural health workforces are as mutually supported as possible.
After Hours is recognised as 6:00pm to 8:00am weekdays and 24 hours on weekends and Public Holidays.
For after hours care, patients are able to do the following:

- In an emergency dial 111;
- Call the practice and follow instructions. The message may direct you to an After Hours Clinic or Accident and Medical Centre or for practices outside of Hamilton, the call may be answered by a triage service on behalf of the practice; or
- Call Healthline on 0800 611 116
Purpose of After Hours service
The goals are to:
- Help improve access to services through reducing financial barriers;
- Address the national variability in fees for after-hours service provision;
- Reduce the number of young children presenting to Emergency Departments with conditions that might be better treated by primary care providers; and
- Enhance child health outcomes.
Patient Portal
Hauraki PHO in partnership with Manage My Health™ (MMH) makes available a Patient Portal to all Hauraki PHO enrolled patients to empower patients to be informed and involved in their care.
The Patient Portal can be used to deliver better care to patients. The Patient Portal offers a secure messaging system between the patient and general practice. The online messaging system provides a high level of security that regular email cannot provide. In addition to secure communication, Patient Portal offers many other features that practices can selectively activate on their own.
Some of the features available are:
- View health information;
- Book non urgent appointments;
- Request repeat prescriptions;
- Secure email to and from health professional or general practice (clinical or administrative);
- Receive Recall reminders;
- Track results and manage health goals; and
- Group messaging (to Patients),
A Patient Portal offers benefits to both patients and general practices. It is secure and convenient (available 24/7) and supports improving customer service to patients by reducing call volumes and waiting times with the ability to book online appointments, receive test results online (once signed off by the doctor) and send and receive non urgent emails. Practices are able to streamline their processes and reduce costs by improving efficiencies.
To sign up for the Patient Portal, please visit your general practice. You will need your own email address. The Patient Portal is available to 16 years and over. If you are under 16 or wish to sign up your children, please discuss this with your general practice.

Hauraki PHO first launched the Shared Electronic Health Record (SEHR) initiative in December 2016. This programme enables a secure electronic summary of patients health information from their general practice to be shared with other health professionals who are directly involved in their care.
The SEHR allows authorised health care professionals including after hours general practices, hospital and emergency department doctors and nurses, access to a summary of information from the patient’s usual doctor, should they need to visit them for care.
Information that will be available includes:
Health professionals will have access to up to date records to help them to provide safe care at any time even if when the patient’s medical practice is closed. Information is stored securely and all access to a patient’s health information is recorded and checked regularly. The SEHR has been in use in the Waikato for some time. There are very strict privacy rules in place to ensure access is strictly limited to health professionals that are directly involved in a patient’s care.
It is important to note that patients are entitled to ‘withhold’ parts of their medical record if they wish and/or opt off this programme entirely. Opt off forms are available at each Hauraki PHO general practice.
